As a CPN, you will play a vital role in providing high-quality mental health care and support to individuals within the community. Key Responsibilities: Conduct comprehensive assessments of individuals' mental health needs and develop personalized care plans. Provide ongoing support, counseling, and interventions to promote recovery and wellbeing. Collaborate with multidisciplinary teams, including psychiatrists, psychologists, social workers, and support workers, to ensure holistic care. Deliver evidence-based interventions and psychoeducation to empower individuals in managing their mental health. Monitor and evaluate individuals' progress, adjusting care plans as necessary to achieve optimal outcomes. Advocate for individuals' rights and access to appropriate services and resources. Participate in crisis intervention and risk assessment, providing timely support and intervention as needed. Requirements: Registered Nurse (RMN) with valid NMC registration. Previous experience working in mental health settings, preferably within community services. Strong assessment, communication, and interpersonal skills. Ability to work autonomously and as part of a team. Commitment to promoting recovery-focused and person-centered care. Knowledge of relevant legislation and policies governing mental health care.
