Location: Monmouthshire (with some office attendance in Newport) Contract: Permanent, full-time, Monday to Friday, 9am to 5pm, 37 hours per week Salary: 25,606 About the Role As an Assertive Outreach Support Worker, you'll: Provide intensive, person-centred support to individuals in temporary accommodation such as B&Bs, hostels, and night shelters. Reach out to those rough sleeping, helping them access the services and support they need. Carry out needs and risk assessments, developing tailored support plans with each person. Work closely with partner agencies (e.g. housing, health, police, and social services) to ensure a joined-up approach. Help people to build support networks and prevent tenancy breakdowns. Be part of a wider team committed to ending homelessness in Monmouthshire. This role is varied, challenging, and rewarding - no two days are the same, but every day you'll be making a positive impact. About You We're looking for someone with: At least 1 year's experience supporting people affected by homelessness or in a related support setting. Strong communication and relationship-building skills, with the ability to engage people who may be mistrustful of services. The ability to work effectively with multiple agencies and partners. Good IT and organisational skills. A full driving licence and access to a vehicle (essential for this role). You'll be empathetic, resilient, and able to stay calm under pressure.
