What you will do:

• Facilitate timely and safe discharges from hospital environments for adults, while managing unplanned situations at home.
• Conduct a variety of assessments including functional, Care Act, safeguarding, Mental Capacity Act, and Continuing Health Care assessments.
• Drive strength-based and person-centred practices, enabling people to thrive in their community by utilizing and building upon their own networks and strengths.
• Collaborate with a multidisciplinary team including Social Workers, Community Assessment Officers, Reablement Coordinators, and Occupational Therapists.
• Form successful partnerships with acute, primary, and community health care sectors, alongside voluntary and independent organisations, to promote health and well-being in line with the Care Act.
• Deliver personalised care plans tailored to adults' care and support needs, ensuring adherence to national legislation and safeguarding principles.
