Assertive Homelessness Outreach Worker - Monmouthshire

Helping people move from crisis to independence

Are you passionate about making a real difference to young people experiencing homelessness? We're looking for a dedicated Assertive Outreach Support Worker to join our team in Monmouthshire. This is an opportunity to directly support individuals who are rough sleeping or in emergency accommodation to take the next steps toward stable, independent living.

Location: Monmouthshire (with some office attendance in Newport)

Contract: Permanent, full-time, Monday to Friday, 9am to 5pm, 37 hours per week

Salary: 25,606

About the Role

As an Assertive Outreach Support Worker, you'll:

  • Provide intensive, person-centred support to individuals in temporary accommodation such as B&Bs, hostels, and night shelters.
  • Reach out to those rough sleeping, helping them access the services and support they need.
  • Carry out needs and risk assessments, developing tailored support plans with each person.
  • Work closely with partner agencies (e.g. housing, health, police, and social services) to ensure a joined-up approach.
  • Help people to build support networks and prevent tenancy breakdowns.
  • Be part of a wider team committed to ending homelessness in Monmouthshire.
  • This role is varied, challenging, and rewarding - no two days are the same, but every day you'll be making a positive impact.

About You

We're looking for someone with:

  • At least 1 year's experience supporting people affected by homelessness or in a related support setting.
  • Strong communication and relationship-building skills, with the ability to engage people who may be mistrustful of services.
  • The ability to work effectively with multiple agencies and partners.
  • Good IT and organisational skills.
  • A full driving licence and access to a vehicle (essential for this role).
  • You'll be empathetic, resilient, and able to stay calm under pressure. Most importantly, you'll share our commitment to giving people the tools they need to rebuild their lives.

What else is on offer?

  • Career Growth Opportunities: Many of our senior leaders began their journeys in entry-level roles, showcasing our commitment to internal progression.
  • Competitive Benefits Package: Enjoy up to 40 days of holiday (including bank holidays), ensuring a healthy work-life balance.
  • Enhanced Group Pension Scheme: Benefit from a generous 10% pension contribution after the qualifying period.
  • Cycle to Work and Eye Care Voucher Scheme: Supporting your health and well-being both at work and in your personal life.
  • Wellbeing and Counselling Support: Access free internal wellbeing and counselling services.
