About the RoleAs part of the Transitions Team, you will work with young adults (typically aged 16-25) who are moving from children's to adult services. Your focus will be on assessing needs early and supporting people to live as independently and confidently as possible.In this role, you will:
Undertake holistic assessments of need, risk, and capacity
Support care planning for young adults with physical, learning, and mental health needs
Work in partnership with families, carers, health professionals, education providers, and voluntary agencies
Promote independence, community inclusion, and choice
Provide person-centred support that leads to sustainable positive outcomes
Use legislation such as the Care Act 2014, Mental Capacity Act, and Children and Families Act to guide best practice
? What You'll NeedWe're looking for an experienced and compassionate practitioner who can balance professional expertise with empathy and creative thinking.You must have:
A Social Work Degree, DipSW, CQSW, or equivalent qualification
Registration with Social Work England
Minimum of 4 years post-qualified experience, preferably in transitions or adult services
Strong knowledge of adult social care legislation, including Care Act 2014 and Mental Capacity Act
Experience working with young people with disabilities, complex needs, or mental health issues
Excellent communication, negotiation, and person-centred planning skills
A track record of working in multi-disciplinary, interagency settings
Confidence in using assessment tools and case management systems
Key Responsibilities
Carry out detailed assessments of young adults' care and support needs
Develop and implement individual support plans that promote independence
Manage a varied and evolving caseload
Provide advice and mentoring to unqualified staff and students
Engage effectively with families and support networks to co-produce care plans
Safeguard vulnerable adults and manage risks effectively
